Physical Evaluations & Sports Physicals
Most schools require a physical exam before participation in sports or physical education programs. Our providers offer:
- School Physicals for general wellness and school clearance
- Sports Physicals for athletic programs — including required forms
- Review of growth, development, and any health concerns

Back to School Services What We Offer
School Immunizations
Protect your child and the school community with up-to-date vaccinations. We offer all required vaccines based on Washington State school requirements, including:
- DTaP/Tdap
- MMR

- Varicella (Chickenpox)

- Meningococcal and HPV (for middle/high school students)

Medication Authorization Forms

- Medication Authorization Form Completion
